ORG14 Diagnosis Is Not a Functional Assessment

A diagnosis can be important information, but it does not tell staff exactly how a person functions in everyday life.

Five things to consider

Diagnosis describes a condition

It may identify a health condition without explaining its practical impact.

Function varies

People with the same diagnosis can have very different abilities and barriers.

Context matters

Lighting, noise, familiarity, fatigue and task demands can change performance.

Real activities matter

Support should be informed by what the person actually needs to do.

Keep clinical information in context

Clinical information can inform support without replacing functional understanding.

Turning this into practical action

✓  Ask what the condition means for everyday tasks.

✓  Record abilities and barriers alongside diagnostic information.

✓  Avoid predicting function from a label alone.

✓  Review functional needs when circumstances change.

What this can look like in practice

Two people recorded as having macular degeneration may require completely different approaches to reading, meal preparation, communication and orientation. The diagnosis cannot supply those details.

Question for your organisation

If you removed the diagnosis from the record, would the remaining information still tell staff how the sensory difficulty affects the person’s life?

Important boundary

This resource does not minimise the importance of diagnosis or clinical care. It explains why clinical information and functional assessment answer different questions.

Want to take this further?

Combining appropriate clinical information with functional understanding produces more usable support. If this resource has highlighted gaps in your organisation’s sensory practice, Sensory Visionaries can help, from reviewing what you currently have in place to providing a structured approach through the Sensory Compliance System.
